We prioritize meaningful relationships and collaborative learning, breaking away from traditional, systems-driven models. During this session, you will learn how to use free Transition Discoveries resources and tools to increase the voices of youth and families to drive successful transition journeys.
During this webinar attendees will:
– Understand Transition Discoveries’ unique approach to youth transition by exploring the power of meaningful relationships in family support and learn about collaborative learning and its impact on empowering families through Discovery Cards.
– Hear from youth and families themselves through lived experience & storytelling.
– Explore a wealth of resources designed to support families, educators, and advocates in empowering youth through transition. From a free resource center to an interactive Discovery Journey designed for person-centered planning, learn how these tools can create a supportive environment for families, enhancing your ability to navigate and empower youth.
Presenters:
Dr. Joan Kester: In her capacity as Program Director of the Interdisciplinary Secondary Transition Program at GW, Dr. Kester leads scholars from across the country, imparting evidence-based secondary transition practices and shaping them into future leaders in the field. With a Doctor of Education degree from the GW Executive Leadership Program in human and organizational learning, Dr. Kester brings a wealth of academic insight and practical experience to her role.
Erin Black: Erin Black, a dynamic young disability advocate and team member at Transition Discoveries, holds a degree in Disability Studies and Public Relations from the University of Toledo. As a recent graduate living with a rare neuromuscular condition, Erin is dedicated to empowering youth with disabilities.
